The failure of a piece of equipment at Pre-Stressed Industries is blamed for the death of a worker this morning.
The 24-year-old man who died was a member of LiUNA Local 625.
Business Manager Rob Petroni says he got the call immediately this morning and says he and the man's co-workers are in shock.
PSI has shut down operations for the day and the Ministry of Labour has sent two investigators and an engineer to the scene.
Petroni calls this a "freak" accident, saying "a stressing machine that is under a substantial amount of PSI let go, it malfunctioned. It's supposed to ease off on the pressure and it didn't, it snapped"
He adds, this was not equipment that had a history of trouble "to my knowledge there hasn't been any concerns with it. It always worked properly and we're looking into obviously ways now to do it differently. But no, to answer your question, there were no concerns"
The union will have grief counsellors at on hand tomorrow.
A second worker was injured, but those injuries are considered non-life threatening.
