Saskatchewan's coroner's system is being reviewed after the Humboldt Broncos bush crash.
A coroner involved in the incident says it wasn't until an injured player woke up in hospital that officials realized there had been a big mistake.
Wayne Nogier also says he ordered the dental records of Xavier Labelle when the young man's name was prematurely released as being among those killed in the crash.
He says before the dental records arrived, Labelle woke up in hospital and had been misidentified as his teammate, Parker Tobin.
Nogier says he doesn't know who made the decision to release Labelle's name as among the dead — only that it wasn't official and he didn't sign off on it.
The junior hockey team had been on its way to a playoff game when its bus and a semi truck collided at a rural intersection - 16 people on the bus were killed.
