The Ministry of Labour has issued three requirements as it continues to investigate a fatal industrial accident in Windsor-Tecumseh.
On April 12, 2018, 24-year-old Michael Cobb of Chatham died after being hurt on the job at Pre-Stressed Systems Inc. on Walker Rd.
The ministry has asked the company to provide an engineering report regarding a piece of equipment, not to use the equipment pending the engineering report and not to disturb the scene.
A ministry inspector and ministry engineer visited the workplace on April 18.
At the time of the fatal industrial accident, an official at LiUNA Local 625--the union representing the workers, said a piece of equipment failed.
LiUNA called it a 'freak accident' and not human error.
