An employee cutting wind turbine flanges suffered a permanent injury when a piece of metal fell, striking the worker.
A ministry investigation determined CS Wind had not provided the proper information and instruction for the worker to do the job safely.
The company was also fined $60,000 in August 2016 after an employee suffered multiple injuries while moving wind turbine components.
The wind turbine manufacturer in east Windsor closed its doors earlier this March.
At its peak, about 500 people were employed at the facility on Anchor Dr.
